What’s a TBI?
If you’re paralized due to a covered accident (limb, partial or full), Balance will pay you a specified percentage of the selected benefit.
100% of benefit for quadriplegia, hemiplegia or paraplegia • 50% for uniplegia
What do these mean?
Loss of Limb
If you lose one or more body members (i.e. arm, leg, hand, foot or eye) resulting from a covered accident, Balance will pay you a specified percentage of the selected benefit.
100% of benefit for loss of two hands, feet, or eyes • 50% for loss of one hand, one foot, one eye, speech or hearing • 25% of benefit for loss of thumb and index finger of the same hand
In the rare case of death, benefits are provided to your family or designated beneficiary.

How do I designate a beneficiary?
$300 a day for up to 60 days in hospital.
(Does not include confinement in a Long Term Acute Care Hospital.)
What’s a Long Term Acute Care Hospital?
$300 per visit payable for treatment in an Emergency Room and $150 per visit for treatment in an Urgent Care Center.
$75 a visit for up to 10 visits with a physical therapist.
Treatment must be certified by a physician.
$500 for ground ambulance and $5,000 for air ambulance transportation.
Your benefits are reduced by 50% if not wearing a helmet at the time of the accident.
